Why Do These Interactions Happen? The CYP450 System

Most drug interactions involving cannabis happen because of the CYP450 enzyme system — a family of liver enzymes responsible for metabolizing (breaking down) most medications. CBD is a particularly potent inhibitor of several key CYP450 enzymes, especially CYP2C9, CYP3A4, CYP2D6, and CYP2C19.

When CBD inhibits these enzymes, medications that depend on them for metabolism accumulate in the bloodstream — their levels rise higher than intended. This can intensify both the therapeutic effects and the side effects of those medications. In some cases (like warfarin), even a modest level increase can be dangerous.

THC interacts differently — its primary interactions are pharmacodynamic (affecting the same body systems as the medication) rather than pharmacokinetic (affecting drug levels). THC's cardiovascular effects (tachycardia, blood pressure changes) are the most clinically relevant for drug interactions.
