Insomnia and sleep disturbance are among the most common reasons adults report using cannabis therapeutically. But the clinical evidence for cannabis as a sleep aid is more nuanced than popular perception suggests — and the specific cannabinoid profile matters enormously.
What the Research Shows
A 2024 systematic review published in Sleep Medicine Reviews analyzed 34 clinical studies examining cannabis and cannabinoids for sleep disorders. The review found consistent evidence that THC reduces sleep onset latency (the time to fall asleep) and increases total sleep time in the short term. However, THC also suppresses REM sleep — the stage associated with dreaming and memory consolidation — which may have long-term implications for cognitive function with chronic use.
CBD's effects on sleep are more complex. At low doses (15–25mg), CBD appears to have alerting properties that may worsen insomnia. At higher doses (150–300mg), CBD has been associated with improved sleep quality in patients with anxiety-related insomnia — likely through its anxiolytic effects rather than direct sleep-promoting mechanisms.
CBN and Sleep
Cannabinol (CBN) has been heavily marketed as a sleep aid, but the clinical evidence is limited. CBN is a degradation product of THC that accumulates as cannabis ages. Early research suggested mild sedative properties, but more recent controlled studies have not consistently replicated these findings. The sedative effects attributed to CBN in older cannabis preparations may have been due to the presence of specific terpenes (particularly myrcene and linalool) rather than CBN itself.
The Terpene Factor
Emerging research suggests that terpenes play a significant role in cannabis's sleep effects. Myrcene, the most abundant terpene in most cannabis cultivars, has demonstrated sedative properties in animal models. Linalool — also found in lavender — has well-documented anxiolytic and sedative effects. Products with high myrcene and linalool content may produce more pronounced sleep effects than cannabinoid content alone would predict.
Practical Guidance
For patients using cannabis for sleep, the evidence supports: (1) THC-dominant or balanced THC:CBD products for sleep onset; (2) higher-dose CBD for anxiety-related insomnia; (3) attention to terpene profiles, particularly myrcene and linalool content; and (4) caution with chronic nightly use due to tolerance development and REM suppression.
