CBD acts primarily as a 5-HT1A serotonin receptor agonist — the same receptor targeted by buspirone and SSRIs — producing anxiolytic effects without the dependence risk of benzodiazepines. CBD also modulates the amygdala's fear response and reduces cortisol. A 2025 JAMA Psychiatry RCT (n=284) found CBD 300mg/day produced a 54% responder rate vs. 31% placebo for GAD.
CBD's primary anxiolytic mechanism. 5-HT1A receptors in the raphe nuclei and hippocampus regulate serotonin tone. Activation reduces anxiety, fear, and stress responses — the same mechanism as SSRIs and buspirone, but via direct receptor agonism rather than reuptake inhibition.
